Application-Declaration (on our form) comprising the following:

- Manufacturer's info (name, address, phone, fax)

- Local applicant's info (Agent-of-Record)

- Tax ID and registration of the importer (representative)

- Exact names of the products to certify, including types and models.

- HS codes of the products (Harmonized Customs codes).

User's manual and specifications

Pictures of the product (one or two)

Existing domestic and international certificates and reports (ISO, CE, GMP etc)

Samples (If deemed necessary)

For TR CU Certificate of Conformity, an onsite inspection may be required.
